NARRATIVE:


On March 13, 1997, Sgt. Andrews and I interviewed Koelpin at her residence. The
interview was conducted from 1540 hours until 1600 hours. The following
information was obtained from Koelpin.

Koelpin stated on December 5, 1995, she overheard a portion of Dr. Minkoff's
conversation with an individual regarding a patient, which was being transported to
Columbia Hospital. For additional information see attached copy of Koelpin's
