INVESTIGATIVE REPORT
This investigative report is regarding investigative assistance provided by the Florida Department
of Law Enforcement (FDLE) to the Clearwater Police Department (CPD) who are investigating the
suspicious death of LISA McPHERSON W/F 02/10/1959.
On February 25, 1997, SA Feola submitted a request for prescription medications to Eckerd
Drugs regarding Dr. David MINKOFF DEA # AM1446346 during the time frame November 1,
1995, to December 30, 1995. Several of the caretaker logs obtained via subpoena, indicated
McPHERSON received Chloral Hydrate during the time of her isolation, 18 November to 5
December 1995, at the Fort Harrison Hotel. Chloral Hydrate is schedule IV controlled substance
and must be prescribed by a certified medical practitioner. The request was predicated by the
fact Dr. MINKOFF was contacted by Janet Johnson on the night of McPHERSON's death and Dr.
DAVID MINKOFF was the attending physician when McPHERSON was pronounced dead at
Columbia New Port Richey Hospital.
On Tuesday, March 11, 1997, SA Feola responded to Eckerd Drugs Corporate Headquarters,
8333 Bryan Dairy Road, Largo, Florida and obtained Dr. DAVID MINKOFF's confidential
Prescription Summary Report for the aforementioned time period. The summary report indicates
Dr. MINKOFF prescribed Chloral Hydrate 500mg to LISA McPHERSON on November 29, 1995.
The prescription was filled on 29 November 1995 and contained 30 pills or tablets. The
prescription was filled at Eckerd Store #0028 which is located at 505 South Belcher Road. The
prescription for LISA McPHERSON was issued in name only, as no date of birth or address is
indicated on the report. A customer prescription summary report was also received and verifies
the information listed above.
On March 20, 1997, SA Feola served a subpoena for records regarding Dr. MINKOFF's
prescriptions and other information related to LISA McPHERSON.
On March 24, 1997, Detective (Det) Jorge Carrasquillo responded to the Eckerd Drugs located at
500 Belcher Road to obtain further information regarding the Chloral Hydrate prescription. Det
Carrasquillo advised the prescription was a phone in to Eckerd Store #0028 and no written
prescription was obtained. The third party insurance claim log was obtained from Eckerd Drugs.

log.
This concludes investigative activity regarding Dr. DAVID MINKOFF and clears lead #50.
