INVESTIGATIVE REPORT
This investigative report is regarding investigative assistance provided by the Florida Department
of Law Enforcement (FDLE) to the Clearwater Police Department (CPD) who are investigating the
suspicious death of LISA McPHERSON W/F 02/10/1959.
This investigative report is in regard to investigative lead #12 concerning additional follow-up on
medical records regarding LISA McPHERSON. Dr. Joan Wood indicated the medical records
obtained from Morton Plant Hospital indicate no physical abnormalities regarding LISA
McPHERSON's examination while at Morton Plant hospital on 18 November 1995. The hospital
records were obtained by Detective Sudler of CPD and are maintained in the CPD case file.
The Columbia HCA New Port Richey medical records regarding LISA McPHERSON's admittance
on the evening of 5 December 1995 were reviewed by Doctor (Dr.) Niles and Registered Nurse
(RN) Barbara Schmid during the interview conducted on 14 February 1997. Both medical
professionals indicated McPHERSON's charts have no unusual notations. The Columbia New
Port Richey records show LISA McPHERSON was dead at the time McPHERSON was examined
by Columbia New Port Richey Hospital personnel. Investigators questioned Barbara Schmid
about blood samples drawn on the evening of 5 December 1995. RN Schmid stated she does
not recall who drew LISA McPHERSON's blood sample on the evening of her death. RN
Roxanne Koelpin, RN Theresa Klimaszewski and RN Rick Pemberton were questioned by
Detective Carrasquillo concerning the drawing of blood. No individual interviewed could recall
exactly who drew the blood but Det. Carrasquillo reports either RN Koelpin or RN Klimaszewski
drew the blood sample from McPHERSON.
From reviewing the laboratory reports on LISA McPHERSON's blood samples, laboratory
examination results indicate a date of birth for LISA McPHERSON as 07/20/1950. LISA
McPHERSON's actual date of birth is 02/10/1959. None of the individuals interviewed from
Columbia HCA Hospital have any knowledge who provided the inaccurate date of birth or where it
was obtained. All individuals involved with LISA McPHERSON had no information regarding its
origin. Additionally, RN Schmid indicated to investigators the laboratory would have normally
obtained the date of birth from the test tube samples submitted with the request for laboratory
analysis. The HCA identification template utilized to identify all of LISA McPHERSON's medical

charts do not include McPHERSON's date of birth. Further, RN Schmid stated the admitting clerk
would have obtained LISA McPHERSONs personal information, i.e., name, date of birth, etc.,
from the individuals who transported her to Columbia HCA. No one interviewed could recall who
exactly drew the blood and where the laboratory obtained the date of birth on the final laboratory
reports. The medical records from HCA Columbia New Port Richey Hospital are contained in the
Clearwater Police Department's case file.
